How to get started dispensing with QCP:
When you are ready to start dispensing... here’s what we need:
1. List of Medications: The first step is determining a list of medications you will dispense. We will target inexpensive generics. QCP can provide a list of medications, a Formulary Selection Guide, for your specialty for reference. What medications do you write the most prescriptions for? We require Medication Name, Strength and Pill Count. We want you to be comfortable. We want a list of medications that you know they could be dispensed in two to three weeks. No more. That will generally be 20 to 30 medications. And, that should take care of about 80% of you patients. We don’t want you to start with too much inventory or too many medications. This will be your formulary and you can always add to it. QCP will provide you a detailed quotation of your formulary and a revenue analysis will be provided upon your request.
2. Credit Application: Have customer complete and send with other required information
3. Physician’s state medical license. A copy to QCP is required.
4. DEA License for one of the physicians at the clinic. A copy to QCP is required. The DEA requires that the DEA license we have on file has the same address as the location where we will ship controlled medications.
Send list of medications, credit application, state medical license and DEA to Mark Holmes at QCP: eorders@qcpmeds.com or FAX 920-232-6774. We need a legible copy of the medical license and the DEA license for our records, so also mail these forms to Mark Holmes at QCP, 7560 Lewis Avenue, Temperance, MI 48182.
5. State dispensing license: Some states require a dispensing license (such as Florida). If your state requires a dispensing license it is normally a nominal fee and requires completing a form. QCP can let you know if your state requires a dispensing license.
